All vital record requests require an application. When submitting a vital record application, a drivers license is required to prove identification or relation. If you are submitting a digital request, you must send a picture copy of your drivers license along with the application.

The first certified copy of a vital record is $15, additional copies are $6
If you need a vital record mailed to you, postage fees will be added to all other charges.

If you are getting married and need to file intentions, please be aware that the filing process is extensive and takes time. If the office is busy, they will file your intentions and call you when the next steps can be taken.
